Who might need a care manager?
Care managers work with both adults and children. They provide info about your health conditions and ways to take care of yourself.
You may be a good fit for a care manager if you:
- Make frequent emergency room (ER) visits
- Have had recent hospital stays
- Manage a complex condition
- Are pregnant, especially if you’re high risk
- See two or more providers
- Just got a referral to a specialist
- May need an organ transplant
- Need medicines or supplies your providers ordered
- Want to apply for Arizona Long-Term Care System (ALTCS)
- Need treatment for substance use
Managing chronic conditions?
Some health issues need more care. If you’re managing a chronic condition, a care manager can help you get the treatment you need to feel better.
Conditions that benefit from a care manager
Some conditions you can get help with are:
- Asthma
- Cancer
- Congestive heart failure
- Chronic obstructive pulmonary disease (COPD)

Your care manager works with:

Your care manager usually works with you by phone. They’ll ask about your health and care needs. You can have family and others present, if you’d like them to take part.
