This plan provides physical (medical) and behavioral health care coverage. We serve Elderly and Physically Disabled (EPD) Long Term Care (LTC) members. People get care and services as part of the Arizona Long Term Care System (ALTCS). Our service area is Maricopa, Gila, Pima and Pinal counties. 

With this plan you can get:

  • Your own case manager for help and answers to your questions
  • The care and services you need in your own home, an assisted-living facility or a nursing home
  • A choice of providers from our wide network
  • Hospice care and services
  • Attendant care or adult day care health services
  • Home health services like nursing services, home health aide, and therapy
  • Meal delivery at home
  • Dental services (up to $1,000 per contract year)

What is AHCCCS? 

AHCCCS is short for Arizona Health Care Cost Containment System. It’s the Medicaid agency that serves people who live in Arizona. You’ll see it written as “AHCCCS.” And people call it “access.” AHCCCS offers health services to people who meet certain income and other requirements.  

 

How to qualify 

This plan is for people who qualify for medical and financial reasons and:

  • Are age 65 or older or have a disability
  • Need ongoing services at a nursing facility level of care

You don’t have to live in a nursing home. Many live in their own homes or an assisted-living facility. 

To get a final decision from AHCCCS, you’ll need to complete the application form.

Check your status 

The decision on your health coverage can take up to 90 days. You’ll get a letter from AHCCCS with either: 

  • Approval of your application and next steps
  • Denial of your application and info about how to appeal the decision

Watch your mail 

Your coverage starts as soon as the state approves your application. You’ll get a Mercy Care welcome packet and member ID card in the mail. 

You can choose a health plan after you get approval for coverage. If you don’t choose a health plan, the state will choose one for you.

Before you choose: 

  • Check with your doctor, pharmacy or hospital. Find out if they work with the plan you choose. Or call the plan to find out if your provider is in their network. Mercy Care has a tool to help you find a provider. 
  • Be sure the plan works in your county. You can check the AHCCCS page on health plans. Choose “Health Plans Available for AHCCCS Medical Assistance” for service areas. 

Options for tribal members 

  • Here are some options and facts to know for tribal members:
  • Are you living off tribal lands? If yes, you can join a Mercy Care health plan.
  • Are you living on tribal lands? If yes, you’ll join the Tribal ALTCS Program.
  • Were you living on tribal lands before admission in a facility off tribal lands? If yes, you’ll join the Tribal ALTCS Program.

Tribal members can also get services through:

  • Indian Health Services Facilities
  • Tribally Operated 638 Health Care Facilities
  • Urban clinics: Urban Indian Health Programs

No longer qualify for health benefits? 

You may still qualify to get treatment for mental health, drugs or alcohol use. Certain state and federal grants cover treatment. To learn more, call us at 602-263-3000 or 1-800-624-3879 (TTY 711). Don’t lose your benefits

Already a member? Be sure your contact info is up to date. Then you’ll get notice when it’s time to renew your Medicaid coverage. Need to update your address, phone number or email? Contact your Mercy Care case manager for help. Or call 1-888-621-6880
